  • Clients will be asked to fill out an intake form prior to the first session. This helps us get to know your goals for therapy. It also helps us determine the proper 'diagnosis' which is required by insurance companies.

  • There may be a fee-for-service such as phone calls, texts, emails, or writing reports or letters on behalf of the patient.

  • Payment is required at the time of service. This includes co-payments, co-insurance, deductible payments or the total self-pay amount if you are not covered by insurance.

  • Your confidentiality is VERY important to us. PLEASE NOTE that state law requires your therapist to report all cases of abuse and neglect to minors or vulnerable adults, and all cases in which there exists danger to you or others.

  • General feedback and treatment progress may be reported to the parents/guardians of children under 14 years of age. Children 14 years of age or older must provide written consent for their therapist to share information with parents/guardians. **More information on confidentiality can be found on the Notice of Privacy page of your New Patient Paperwork.

  • Questions about your treatment should be directed to your therapist.

  • Questions about office practices should be directed to the APP Office Manager.
